Guide for GP Referrals
STEP 1
Complete this contact page with your patient's information.
STEP 2
Write a referral letter including:
- Type of plan
- Number of sessions for individual or group therapy (up to 10 per calendar year)
- Signature electronic or otherwise.
STEP 3
Share your letter (and Medicare Plan):
- Upload through the Upload File button;
- Fax to 02 8915 1511, or;
- Supply your patient with a hard copy.
STEP 4
Let your patient know our admin team will contact them within 2 business days of this request. They can expect a phone call, text or email from us.
