STEP 1
Select the assessment you are seeking. If you require more than one assessment, please indicate this in your message.
STEP 2
If you are under the age of 25 years and have a referral from a paediatrician or psychiatrist for ASD testing, please:
- Upload it to the File Upload button, or;
- Fax to 02 8915 1511 .
STEP 3
Our admin team will be in contact to discuss your request and book your assessment.
